- Suffering from an illness (e.g. ‘flu).
- Under stress.
- Fatigued.
- Under the influence of drugs or alcohol.
- Past personal experiences that is skewing their thinking.
- Lack of good quality training and education on the hazards and risk of the job.
- PPE that is interfering with their ability to detect the hazards creating the risk.
- Workplace conditions, such as high levels of noise, may be interfering with their ability to correctly identify the hazard that creates the risk.
